This program is intended to expand access to COVID-19 risk screening and testing. It is not intended for people experiencing severe symptoms such as severe cough, severe shortness of breath, severe fever, or other concerning symptoms who may need more immediate medical care. Please visit the CDC website for information on what to do if you are sick.
This is a FREE state supported test collection site operating at no cost to the individual. You do NOT need symptoms to test.

Pre-Register for your COVID-19 Test
Pre-registration is encouraged by clicking the button below and following the provided steps. Once registered with our laboratory, our staff can quickly expedite sample collection at any of our COVID-19 testing locations throughout the state. If you are having difficulty and need assistance, please contact our Customer Service team at 1-888-998-3568.
Step 1: Click the Button or go to https://cov19.health/#/ and register as a “new patient”
Step 2: Type in SMGCOMC & Complete log-in information
Step 3: Select “Client Bill” – Test is FREE; no insurance required
Step 4: Once submitted – verification via email and/or text
